Aid and Attendance is a VA pension addition

VA describes Aid and Attendance and Housebound as monthly payments added to a qualifying Veteran’s or survivor’s monthly pension. Aid and Attendance may be available to a pension recipient who needs help with daily activities, is bedridden, is a nursing-home patient because of disability, or meets the stated visual-impairment criteria; VA says the person cannot receive Aid and Attendance and Housebound at the same time (VA Aid and Attendance guidance).

Oklahoma has an unusually direct SoonerCare rule for this benefit. In a nursing facility, OHCA treats VA Aid and Attendance or Champus as a third-party resource whether paid to the individual or facility and says it does not affect income eligibility or the vendor payment. When the actual Aid and Attendance payment is for an attendant in the individual’s home, the rule treats it as excluded income. That distinction should be documented in the Medicaid budget rather than assumed from the VA award letter alone (OHCA VA Aid and Attendance income rule).

VA’s $90 rate and Oklahoma’s nursing-facility budget

VA has a separate $90 pension-rate rule for a person with no dependents who lives in a Medicaid-covered nursing facility and meets VA’s conditions. VA says the facility cannot count that $90 monthly payment toward the cost of care and the person keeps it for personal expenses; the payment replaces disability compensation or DIC in the stated circumstance (VA $90 Medicaid-facility pension guidance).

OHCA’s own rule aligns with that concept: it says the reduced VA pension is not used to compute vendor payment or spenddown, and the resident receives the $90 reduced pension plus the regular nursing-facility maintenance standard. Appendix C-1 currently lists the ordinary nursing-facility maintenance standard as $75 per month. A veteran’s other VA pension, compensation, dependents, and facility status still require individual review (OHCA VA-income policy; OKDHS Appendix C-1).

Seven Oklahoma State Veterans Homes and a homestead benefit

The Oklahoma Department of Veterans Affairs currently lists seven State Veterans Homes: Ardmore, Claremore, Clinton, Lawton, Norman, Sallisaw, and Sulphur. Its page identifies the homes as federally approved by VA and licensed by the Oklahoma State Department of Health; the listed availability is informational, so a resident or family should contact the admissions team for the live admission and payment requirements (Oklahoma Veterans Homes).

Oklahoma’s 2026 Form 998 says a qualifying head of household can claim an exemption for the full fair-cash value of the homestead. The form requires Oklahoma residency, qualifying homestead status, honorable discharge, VA certification of a 100% permanent qualifying disability or compensation at the 100% rate, and ownership and occupancy on January 1; it also covers a qualifying surviving spouse. This tax relief does not change a Medicaid resource or estate-recovery determination on its own (Oklahoma 2026 Form 998; OHCA recovery rule).
