ADvantage is Oklahoma’s key aging-at-home waiver

ADvantage is Oklahoma’s Medicaid Home and Community-Based Services waiver for older adults and a targeted group of physically disabled adults who would otherwise be at risk of needing long-term-care-facility services. OHCA describes it as a way to finance noninstitutional long-term care, and the program’s public page lists personal care and other supports for eligible members (OHCA ADvantage services rule; OHCA ADvantage overview).

The applicant must qualify financially for SoonerCare, be age 65 or be a qualifying disabled adult, and be determined by Aging Services nursing staff to meet nursing-facility institutional level of care. The public page also requires the person to live in the person’s own or a family member’s home, have needs safely met with waiver services and family or community supports, and not have the listed intellectual-disability or cognitive-impairment exclusions. The right waiver is determined by the diagnosis, care setting, and level of care, not simply by age (OHCA ADvantage eligibility page).

A waiver slot is required

ADvantage is not an unlimited entitlement once financial and medical standards are met. The program rule says the number of people who may receive services is limited and that an open waiver slot authorized by CMS must be available for federal participation. If all slots are filled, the person cannot be certified as eligible for ADvantage and is placed on a waiting list until an open slot becomes available (OHCA ADvantage waiting-list rule).

CMS’s waiver page identifies the Oklahoma ADvantage waiver as a Section 1915(c) program and shows its then-current approval period through June 30, 2026. A family should verify the new waiver approval and current capacity when applying because a federal waiver renewal, a published enrollment ceiling, and local access can change over time (CMS Oklahoma ADvantage waiver record).

Home care still uses long-term-care eligibility rules

For a person receiving ADvantage for 30 days or longer, OHCA uses the categorically-needy standard in Appendix C-1 Schedule VIII.B. The current table gives that income standard as $2,982 monthly, while an individual’s published resource standard is $2,000. A Medicaid Income Pension Trust may be relevant when countable income exceeds the standard but is within the permitted MIPT range; actual eligibility needs a case calculation (OHCA ADvantage financial rule; OKDHS Appendix C-1).

ADvantage also uses the same transfer-of-assets framework that can produce a service penalty. A household that hopes to keep someone home should plan both the immediate private-pay back-up and the possibility that a waiver place will not be immediately open, rather than assuming that an approved financial application guarantees services on a chosen date (OHCA ADvantage transfer rule; OHCA ADvantage capacity rule).

Oklahoma home-care reality: ADvantage requires SoonerCare eligibility, nursing-facility level of care, and an available CMS-authorized slot. A waiting list is written into the program rule (OHCA ADvantage services rule).
