Iowa Medicaid is the long-term-care program

Iowa Medicaid is the state-federal program used for long-term services and supports. Iowa HHS describes LTSS as services that help a Medicaid member maintain independence at home or, when needed, in a facility. The agency's institutional manual identifies the Iowa Department of Health and Human Services and Iowa Medicaid as the entities operating the relevant eligibility and payment processes (Iowa HHS LTSS overview; Iowa HHS Medical Institutions manual).

For a person entering a nursing facility, financial eligibility is only one part of the determination. The institutional manual requires the applicable income and resource rules, an institutional level-of-care need, and, for the 300% income group, a 30-consecutive-day institutional stay before approval. Facility, waiver, and PACE routes are different

Iowa's published rules address nursing facilities, intermediate-care facilities for people with intellectual disabilities, psychiatric medical institutions, HCBS waivers, and PACE. The community routes are not simply a nursing-home benefit delivered at home: waiver applicants need financial eligibility and the applicable functional or institutional level-of-care determination (Iowa HHS Medical Institutions manual; Iowa HHS HCBS waiver overview).

The Facility and Waiver Eligibility Team processes applications for long-term-care facility and HCBS-waiver coverage, according to Iowa HHS application assistance material. That is useful operational information, but a complete application still needs the household's income, resources, transfer history, insurance, marital facts, and functional evidence (Iowa HHS LTSS application assistance).

The MAIT confirms Iowa is an income-cap state

Iowa permits a Medical Assistance Income Trust when income exceeds 300% of the one-person SSI benefit. HHS also states that a person using a MAIT must have total gross monthly income no higher than 125% of the applicable statewide average charge; the current nursing-facility amount shown by HHS is $12,002.50 for July 1, 2026 through June 30, 2027 (Iowa HHS MAIT guide).

Program labels do not decide eligibility on their own. A family should ask Iowa HHS which route applies and obtain the current written calculation before making a transfer, entering a facility, or funding a trust.
