Iowa uses an expanded recovery definition
Iowa HHS describes estate recovery as the state's post-death claim for Medicaid costs, including managed-care capitation payments, subject to the program rules. The agency's policy applies recovery to members age 55 or older and to certain permanently institutionalized members under 55, and states that recovery cannot exceed the amount paid (Iowa HHS estate recovery overview; Iowa HHS Medicaid Resources manual).
The policy's estate definition reaches all real property, personal property, and other assets in which the member had legal title or an interest at death. It specifically includes jointly held property, trusts, and retained life estates. That is an expanded-recovery structure, not a rule confined to a narrow probate-only estate description (Iowa HHS Medicaid Resources manual).
