Aid and Attendance is a federal pension add-on

VA Aid and Attendance is not a separate District program. VA says it is a monthly payment added to a qualifying Veteran’s or survivor’s VA pension. A pension recipient may qualify when another person is needed for daily activities such as bathing, feeding, or dressing; when the person is bedridden; when the person is in a nursing home because of disability-related mental or physical loss; or when the stated visual criteria are met. VA also says Aid and Attendance and Housebound benefits cannot be received at the same time (VA Aid and Attendance and Housebound benefits).

VA directs nursing-home applicants to include VA Form 21-0779 and generally directs mail applicants to VA Form 21-2680, completed in part by a medical examiner. Benefit rates depend on the current pension rate and the claimant’s income for VA purposes, so this page does not use a fixed dollar figure. District veterans-home and tax context

VA explains that State Veterans Homes are state-owned, state-operated facilities providing nursing-home, domiciliary, or adult-day care, and says the network is in all 50 states and Puerto Rico. The official materials retrieved here did not identify a District-operated State Veterans Home, so this page does not claim that DC has one; use VA’s facility locator and the DC Office of Veterans Affairs for current local options and referrals (VA State Veterans Homes; DC Office of Veterans Affairs).

The District does offer a Disabled Veterans Homestead Deduction. Its current page says a qualifying Veteran with a total and permanent service-connected disability, or paid at the 100% rate because of unemployability, may receive a $445,000 assessed-value reduction for a qualifying principal residence. For tax year 2026, it lists a $163,500 household-income limit, at least 50% ownership, District domicile, and occupancy conditions; it also says the property cannot receive certain other named District tax benefits at the same time (DC Disabled Veterans Homestead Deduction).

Medicaid budgeting must be checked directly

A VA award does not replace a District Medicaid financial determination. DHCF’s current long-term-care page identifies income and resources as part of eligibility and describes the Special Income Standard and spend-down routes, but it does not give a current Aid-and-Attendance-specific budgeting rule. An older District policy manual mentions a Veteran’s Aid and Allowance personal-needs figure, but it is a 2017 manual with older standards and should not be treated as current policy (DHCF Long-Term Care overview; District ESA policy manual).

Before filing, ask DHCF in writing how the actual VA payment is counted for eligibility and post-eligibility patient liability, and give the caseworker the award letter and proof of any unreimbursed medical expense.
