Community Choices Waiver is Wyoming’s main home-care alternative

Wyoming’s Community Choices Waiver, usually called CCW, is a Medicaid home- and community-based services waiver. The Department says it assists older adults age 65 or older who are on, or eligible for, Medicaid and adults ages 19 through 64 with a confirmed qualifying disability. Its fact sheet identifies CCW as a Section 1915(c) waiver and explains that it offers an alternative to nursing-facility care through defined community-based services (Wyoming HCBS and CCW overview; Wyoming CCW 2026 fact sheet).

The state’s HealthStat report says Wyoming provides long-term care to people meeting nursing-home level of care through CCW and nursing homes. That framing is important: CCW is not simply ordinary Medicaid with extra hours of help. It is a defined long-term-care alternative that uses the state’s functional assessment, financial eligibility process, service plan, and provider network (Wyoming CCW program description; CMS HCBS waiver authorities).

Level of care, finances, and annual review all matter

The current CCW fact sheet says the Department must determine the person needs nursing-facility level of care and that the financial pathway generally requires Medicaid eligibility or the Special HCBS Waiver Group, income at or below 300% of the FBR, and limited countable resources. Wyoming Chapter 34 requires the nursing-facility level-of-care assessment to be completed no more than 365 days before CCW enrollment and requires at least annual re-evaluation (Wyoming CCW eligibility fact sheet; Wyoming Medicaid Chapter 34).

Chapter 22 supplies the standards for nursing-facility level-of-care decisions. This means an applicant should prepare information about functional needs, supervision, medical risks, current supports, and the home setting. A diagnosis by itself is not the published test, and a financial approval does not replace the functional determination (Wyoming Chapter 22 level-of-care rule; Wyoming CCW application information).

CCW has capacity rules, but no current reported CCW waitlist

Wyoming Chapter 34 permits the Department to manage CCW enrollment capacity statewide and, if capacity is reached, to place eligible people on a Department-maintained waiting list in chronological eligibility order. That is an enrollment-cap rule, not a promise that a qualified applicant will always enroll immediately (Wyoming CCW capacity and waiting-list rule).

For the current status, the Department’s July 2026 community-programs report says there is no Comprehensive or CCW waitlist. Its report separately shows waitlist counts for Supports waivers, which is why a generic statement that “Wyoming waivers have a waitlist” would be inaccurate for CCW. Current capacity and service availability still need confirmation at application because a report can become outdated (Wyoming July 2026 HCBS report).
