There is a registry and priority process

LDH says a Community Choices Waiver applicant must meet LTC Medicaid eligibility, be age 21 or older, and meet nursing-home level of care. The agency's fact sheet says it keeps a Community Choices Waiver Request for Services Registry, or RFSR, of people who request services, making the date of request and priority category material to access (LDH Community Choices Waiver; LDH CCW fact sheet).

LDH describes priority groups for Community Choices, including people referred by protective services where services are needed to prevent institutional placement, people with ALS, recent qualifying hospital discharges, and certain nursing-facility residents. Other offers are first-come, first-served by request date, so a person can be eligible yet not immediately receive a waiver offer (LDH Community Choices Waiver).

LDH's Adult Day Health Care Waiver page likewise says applicants must meet LTC Medicaid eligibility, be age 22 or older, and meet nursing-facility level of care. Its provider manual describes a Request for Services Registry as an eligibility condition, confirming that availability and enrollment must be checked in addition to clinical and financial eligibility (LDH Adult Day Health Care Waiver; LDH ADHC provider manual).

Financial rules still apply

LDH's current chart applies a $2,982 individual SIL, $2,000 individual resource limit, and $3,000 couple resource limit to LTC/HCBS. Its transfer policy applies the 60-month review to HCBS waiver applicants and beneficiaries as well as institutional LTC applicants (LDH Z-700, January 2026; LDH I-1670 transfer policy).
