Kentucky Medicaid generally excludes the A&A increment
CHFS's policy distinguishes the Aid and Attendance and unreimbursed-medical-expense amounts from other VA income for a long-term-care recipient. The manual directs workers to exclude VA A&A and VA UME in Medicaid eligibility and patient-liability determinations, while treating them as countable for beneficiaries in state-operated veterans nursing facilities that accept Medicaid (CHFS Volume IVA policy manual).
The manual also says a VA A&A payment is an excluded resource in the month received and the following month, then is countable as a resource afterward. That distinction between income treatment and later resource treatment is an important Kentucky budgeting point (CHFS Volume IVA policy manual).
VA's $90 nursing-facility pension rule may matter
VA explains that a person with no dependents in a Medicaid-covered nursing facility may be awarded VA pension at the $90 monthly rate when otherwise eligible. If awarded, the facility cannot count that payment as income toward the cost of care, and the person retains the full $90 for personal expenses (VA $90 Medicaid-facility pension guidance).
The $90 rate replaces disability compensation or Dependency and Indemnity Compensation rather than adding to them. Kentucky's own nursing-facility personal-needs allowance is $60 a month, so the VA rule should be considered with the resident's individual VA entitlement and Kentucky patient-liability budget (VA $90 Medicaid-facility pension guidance; CHFS Volume IVA policy manual).
Kentucky State Veterans Centers are an additional care setting
Kentucky's official FY23 report describes four State Veterans Centers: Thomson-Hood in Wilmore, Eastern Kentucky in Hazard, Western Kentucky in Hanson, and Carl M. Brashear in Radcliff; it said a fifth was under construction in Bowling Green. Because that report is older, confirm current capacity and operating status directly before choosing a setting (Kentucky FY23 Veterans Affairs annual report).
The current Veterans Centers page says Kentucky operates skilled long-term-care communities, charges a monthly fee based on accommodation, accepts Medicare, Medicaid, private pay, and many insurance policies, and provides full VA per diem with no veteran out-of-pocket cost for veterans rated 70% or more disabled (Kentucky Veterans Centers).
Coordinate, do not assume duplication
VA benefits and Kentucky Medicaid are separate programs with separate financial and care-setting rules. Before changing a VA payment election or nursing setting, obtain the current VA award information and Kentucky patient-liability calculation, particularly if a State Veterans Center accepts Medicaid (VA Aid and Attendance guidance; CHFS Volume IVA policy manual).
Kentucky's general homestead exemption is not veterans-specific: homeowners age 65 or older or classified as totally disabled can qualify under the stated requirements, and the 2025–2026 exempt assessed value is $49,100 (Kentucky homestead-exemption page).
