VA Aid and Attendance is federal, but Hawaii care choices are local

VA Aid and Attendance is a federal veterans benefit, while Hawaii Medicaid eligibility and patient-liability decisions are made under Hawaii’s Med-QUEST rules. The Med-QUEST sources reviewed for this page did not provide a Hawaii-specific rule stating that Aid and Attendance is categorically excluded from long-term-care income or cost share, so families should not assume an exclusion without a current written calculation from Med-QUEST (Hawaii Med-QUEST, QUEST Integration; Hawaii MAGI-excepted income rule).

For a veteran who may need assistance at home, in a care home, or in a nursing facility, keep the VA award letter and a precise list of recurring medical and care expenses. Those records can be needed for a VA claim as well as a separate Med-QUEST eligibility or cost-share review.

Hawaii has two operating State Veterans Homes

Hawaii’s Office of Veterans’ Services identifies the Yukio Okutsu Hilo Veterans Home and the Daniel K. Akaka Veterans Home as State Veterans Homes under its development and oversight. The Office describes the Hilo home as a post-acute and rehabilitation provider with both short-term rehabilitation and residential long-term placement (Hawaii Office of Veterans’ Services benefits and services; Hawaii State Veterans Homes).

The Daniel K. Akaka State Veterans Home is a 120-bed Kapolei facility. VA reports that the Yukio Okutsu State Veterans Home in Hilo opened in 2007 with 95 beds and that the Akaka home is Hawaii’s second State Veterans Home; a third Maui home was still planned rather than operating when VA published its report (VA Pacific Islands Health Care System).

Home-tax relief is county-administered

Hawaii’s Office of Veterans’ Services states that totally disabled veterans or their widow(er) may receive real-property tax exemptions for property owned and occupied as a home. The Office directs veterans to their local real-property tax office because the benefit is administered by island or county, so eligibility and the remaining minimum tax or assessments should be checked locally (Hawaii Office of Veterans’ Services property-tax information).

State Veterans Home eligibility, VA payment, Medicaid coverage, and tax relief are separate programs. Contact the home, a VA-accredited representative, and Med-QUEST before committing assets or benefits to a care arrangement.

The Hawaii Office of Veterans’ Services also describes assistance with preparation of VA claims and VA appeals. That state assistance can be a useful starting point for benefit questions, but it does not replace the federal VA eligibility decision or a Med-QUEST income-and-cost-share determination (Hawaii Office of Veterans’ Services benefits and services).
