An interest list is part of the STAR+PLUS HCBS reality
STAR+PLUS HCBS is capped and non-entitlement, so HHSC maintains an interest list and assesses eligibility when a person's name reaches the top. HHSC describes limited routes that may bypass the ordinary interest-list path, including an upgrade process for certain enrolled people and Money Follows the Person for nursing-facility transitions (HHSC STAR+PLUS HCBS eligibility policy).
Do not rely on old waitlist numbers: a 2022 legislative handout reported 19,723 people on the STAR+PLUS Waiver interest list and a one-to-two-year estimate, but that report is dated and should not be represented as a current wait-time forecast (

Community First Choice is a different coverage route
Community First Choice is a Medicaid State Plan option rather than a capped waiver. Texas authorized the option through State Plan Amendment TX-14-026, and the program provides home-and-community-based attendant services and supports for people meeting Medicaid eligibility and institutional-level-of-care requirements (Texas Community First Choice amendment; HHSC Community First Choice overview).
An HHSC committee presentation reported that eligible Community First Choice participants have no interest list, but that presentation was available only through a search-result snippet in the underlying research. Treat the point as a program distinction to confirm with HHSC for an active case (HHSC 2024 committee presentation).
Choose a path based on need and access
Home care is not a single Texas benefit. STAR+PLUS HCBS and Community First Choice have different structures, financial requirements, and access mechanics, and a person cannot be enrolled in more than one Medicaid waiver at the same time (HHSC STAR+PLUS HCBS eligibility policy).
