Minnesota offers several HCBS routes

Minnesota DHS lists the Elderly Waiver, Brain Injury Waiver, Community Alternative Care Waiver, Community Access for Disability Inclusion Waiver, Developmental Disabilities Waiver, and Alternative Care program among its home- and community-based offerings. Applications go through the local county or Tribal social-services agency, and a screening process determines eligibility for waiver programs (DHS HCBS waiver overview).

The Elderly Waiver is the principal older-adult MA waiver. DHS says it provides home- and community-based services to people who meet nursing-facility level of care, qualify for MA, and choose community living. Counties, Tribal entities, and managed care organizations administer the program, so a family should ask the local lead agency about the assessment, service plan, and provider availability (Minnesota Elderly Waiver).

Waitlists are not the whole access question

DHS reports that Minnesota eliminated the CAC, CADI, BI, and DD waiver waitlists in 2016. It says CAC, CADI, and BI no longer have a waitlist, and that DD waitlists are no longer in effect, though funding must be approved within specified timelines for people determined to need the DD Waiver. This is favorable compared with a formal waiting list, but it is not a promise that every person will immediately receive every requested service (DHS waiver waitlist information).

Financial eligibility still matters. DHS says a person age 65 or older who is above an MA income limit may be eligible through a spenddown, while chapter 256S supplies posteligibility treatment rules for Elderly Waiver participants. The appropriate calculation depends on the person's pathway, income, spouse, and living arrangement, so generic “waiver income caps” are a poor substitute for the actual MA-LTC determination (DHS coverage for people age 65 or older; Minnesota Statutes section 256S.05).

Planning starts with the assessment

DHS says Long-Term Care Consultation helps people make choices that reflect their needs and preferences and identifies services that can help them remain at home. A consultation can therefore provide the care evidence that should be coordinated with MA financial planning, rather than treating home care as a purely private-pay alternative (DHS Long-Term Care Consultation).
