Alabama offers seven HCBS waiver paths

Alabama Medicaid says seven waiver programs provide alternatives to institutional care. CMS identifies the current Alabama set as the Alabama Community Transition (ACT) Waiver, Community Waiver Program (CWP), Living at Home (LAH) Waiver, Intellectual Disabilities (ID) Waiver, Elderly and Disabled (E&D) Waiver, State of Alabama Independent Living (SAIL) Waiver, and Technology Assisted (TA) Waiver for Adults (Alabama Medicaid waiver overview; CMS Alabama waiver factsheet).

Those names are not interchangeable. CMS describes E&D and ACT as serving older people and people with physical or other disabilities who meet nursing-facility level of care, while ID, LAH, and CWP use an intermediate-care-facility-for-individuals-with-intellectual-disabilities standard. SAIL serves qualifying adults with physical disabilities, and TA is for eligible technology-dependent adults (CMS Alabama waiver factsheet).

E&D is the main aging-in-place waiver

The E&D waiver is Alabama's primary senior-focused home-care pathway. Alabama's 2026 HCBS handout states a $2,982 monthly income limit, a $2,000 resource limit on the first day of the month, and a nursing-facility level-of-care requirement; its listed supports include case management, homemaker and personal care, respite, adult day health, companion services, meals, nursing, supplies, PERS, and home modifications (Alabama Medicaid 2026 HCBS handout).

ACT also uses the nursing-facility level-of-care framework and adds transitional-assistance services. The official CMS factsheet describes ACT services such as adult day health, case management, homemaker and personal care, respite, assistive technology, meals, home modifications, skilled nursing, and transitional assistance; a family returning from an institution should ask specifically whether ACT rather than E&D fits the transition need (CMS Alabama waiver factsheet).

Enrollment warning: Alabama Medicaid says waiver enrollment is limited and a waiting period may be necessary. One current E&D summary reports a 15,000-person annual maximum and 25 nursing-facility transition slots, but capacity and queue status need direct confirmation when applying (Alabama Medicaid HCBS overview; E&D waiver enrollment summary).

Waitlists change a funding plan

Unlike institutional Medicaid, a waiver may not begin immediately simply because a person qualifies. The Alabama E&D waiver summary says it is not an entitlement and that waiting lists form when participant slots are full. That means a household should consider interim unpaid caregiving, private-pay help, other covered services, and facility alternatives while keeping the waiver application moving (E&D waiver enrollment summary).

Applications should be matched to the correct administering agency. Alabama's handout directs E&D inquiries to the Department of Senior Services and SAIL inquiries to the Department of Rehabilitation Services, while the Agency identifies the Department of Mental Health as the CWP operator.
