Financial rules remain important at home
The current TennCare LTSS guide gives a 2026 income cap of $2,982 per month and a $2,000 countable-resource limit for the ordinary LTSS pathway. TennCare’s QIT policy applies to individuals who seek nursing-facility or HCBS LTSS and have income over the cap, provided the other eligibility and trust requirements are met. That makes an income trust a potential home-care planning issue as well as a facility issue. TennCare QIT policy
Enrollment targets and limited slots
TennCare says Group 3 applicants must be at risk of nursing-facility care and either receive SSI or meet the financial rules and have an available slot. For people with intellectual or developmental disabilities, Employment and Community First CHOICES is a separate program with services designed to support community living. TennCare states that some ECF CHOICES groups can enroll immediately based on urgent needs while other groups receive services only when a slot is available, and it calls those slots very limited. Level of care and service planning
CHOICES requires nursing-facility level of care for Groups 1 and 2 and an at-risk level for Group 3. A PAE and the person-centered service plan therefore matter alongside the financial application when someone seeks care at home. TennCare CHOICES Tennessee LTC Partnership guidelines
Ask the TennCare plan or local aging and disability contact which group is being considered, whether a slot is open, what clinical determination is needed, and how a later change in condition will be handled.
