Oregon's home- and community-based pathways

Oregon's Aging and People with Disabilities program describes two Medicaid waivers approved by CMS through 2026: the 1915(c) Aged and Physically Disabled Waiver and the 1915(b)(4) Case Management and Agency with Freedom of Choice Waiver. ODHS says these waivers help people receive assistance at home or in the community (ODHS APD waivers and K Plan).

CMS lists the Oregon Aged and Physically Disabled Waiver as approved, effective January 1, 2022, and expiring December 31, 2026. CMS describes it as providing waiver case management for people age 65 and older and physically disabled people ages 18 through 64 (CMS Oregon APD waiver record).

ODHS says it is renewing the two APD waivers and that their current approvals expire December 31, 2026 unless renewed. The public page notes an expected increase in users of case-management services because of rising caseloads, but it does not publish a current statewide numerical waiting list or enrollment cap (ODHS APD waivers and K Plan).

The K Plan is a state-plan option, not the same waiver

Oregon describes the K Plan as a Medicaid state-plan option authorized by the Affordable Care Act. It supports home- and community-based services and supports for people who need regular help and want to remain independent, healthy, and safe at home or in community settings (ODHS K Plan overview).

ODHS says the K Plan gives participating states a six-percent increase in federal medical assistance funds for the covered services. That financing description does not by itself establish eligibility for any specific Oregon service; the individual must still meet the applicable OHP and service criteria (ODHS K Plan overview).

Functional eligibility is an Oregon service-priority decision

When applying for Medicaid long-term-care services, Oregon evaluates the help a person needs with activities of daily living. ODHS calls the resulting measure a service priority level, numbered 1 through 18, and says the information is used to decide eligibility for services (ODHS ADL and service-priority guidance).

The applicant must also meet income guidelines, and ODHS says funding is limited. The located state materials do not state one public statewide waiting-list number for the APD waiver, so families should ask their local ODHS/APD contact about current availability, assessment timing, and the specific service requested (ODHS ADL guidance; ODHS waiver page).
