Ohio's confirmed Average Private Pay Rate is $7,787 per month, effective September 1, 2024. ODM's MEPL explains that the rate is recalculated every two years by the Scripps Gerontology Center at Miami University for the Ohio Department of Aging (ODM MEPL No. 181).
The penalty period is based on the uncompensated value transferred divided by that monthly APPR. Under the cited rule framework, the period begins no earlier than the later of the transfer date or the date the person is otherwise eligible and would receive long-term-care services but for the penalty (OAC 5160:1-6-06; ODM MEPL No. 181).

Some transfers are exempt or can be challenged
Ohio's rule lists exempt transfer categories, including transfers to a spouse, a minor or blind or disabled child, and certain transfers for a purpose other than qualifying for Medicaid. The statute and rule framework also recognize specified sole-benefit trusts and home transfers involving qualifying caregivers or siblings (OAC 5160:1-6-06).
An undue-hardship exemption may be available when a penalty would deprive the person of necessary medical care or basic necessities such as food, clothing, or shelter. This is a rule-based exception, not an automatic result of financial difficulty (OAC 5160:1-6-06.6).
Do not treat the calendar as a gifting checklist
A transfer can affect Medicaid, taxes, creditor exposure, VA benefits, and a spouse differently. Preserve records of value, purpose, and timing, and seek Ohio-specific legal advice before making a transfer intended to affect Medicaid eligibility.
