Alaska applies a five-year transfer review

The current Alaska long-term-care eligibility guide states that Nursing Home Medicaid and HCBS waiver applicants face a 60-month, or five-year, look-back immediately before application. It says a transfer for less than fair market value can lead to a period of Medicaid ineligibility, while the ABD Medicaid pathway does not use that long-term-care look-back (2026 Alaska long-term-care eligibility guide).

A 2025 Alaska elder-law presentation describes the review as reaching back five years from the later of institutionalization or the LTC Medicaid application date and cites ADLTC 554D. It also says the state presumes a transfer by the applicant or spouse was made to qualify, stay eligible, or avoid recovery unless the client rebuts that presumption with convincing evidence (Alaska LTC Medicaid eligibility presentation).

Alaska does not use one statewide published divisor

Alaska's distinctive penalty rule is local. The 2025 presentation says the uncompensated value is divided by the local skilled-nursing-facility Medicaid daily payment rate where services will be received; if there is no local skilled nursing facility, the rate is the statewide swing-bed rate. It says new rates are issued each January (Alaska LTC Medicaid eligibility presentation).

That approach means it is inaccurate to present one statewide Alaska 2026 transfer divisor as though every applicant uses it. The presentation's 2025 examples show different daily rates for Anchorage, the Mat-Su Valley, and Homer, demonstrating why the actual facility and location matter. The penalty is a care-coverage delay

The same presentation explains that the penalty is a time penalty, not a bill for a fixed dollar amount. It begins when the person is otherwise Medicaid eligible and would receive institutional-level services except for the transfer penalty; it has no stated cap, and a transaction within the review period can create a calculated ineligibility period longer than five years (Alaska LTC Medicaid eligibility presentation).

Do not confuse a tax-free gift amount with a Medicaid-safe transfer. The Alaska presentation specifically says that the federal annual gift-tax rule does not create a comparable Medicaid exemption.
